Hello ABF Community,

Arizona Burn Foundation is looking for feedback from burn survivors and their families regarding participation in our Burn Survivor Peer Support Group. In the past, Support Group was held bi-weekly, on Wednesdays from 5:30pm-7pm. Due to low attendance, ABF would like to know if a support group is wanted by survivors and loved ones and what changes could be made to increase attendance. This group is designed for survivors over the age of 18. If you’re the guardian of a survivor under 18 years old, and would like them to attend, please leave the questions blank and scroll to the bottom and put your request for a children’s group in the text box (#7)

If you are not interested in the Support Group, please answer “no” to question #2 and N/A to the rest of the questions.
